What to Expect
In helping you understand the process of the Second Opinion Consultation,
we have provided a brief description of each phase. The cost for a Complete
Second Opinion is $350. You will
receive a complete analysis based on your submitted medical history.
The first step in the Second Opinion Consultation process
is to complete a confidential questionnaire disclosing information about your
family history, personal health history, medical
background, medication,
child bearing history, and prior treatment results.
Analysis
We will thoroughly review your medical medical history. If we require additional
information, we will contact you. If you have additional information for us,
you can e-Mail the Seond Opinion
Coordinator or Fax us the information at 212-994-4499.
Recommendations Provided to You
Based on your medical profile and family history we will
provide to you, within one week, with our Second Opinion.
After this consultation, if you pursue further services from CHR, we will
discount those services at the costs for this initial consultation. The options
may include further consultations with us by phone, in-person, or a referral
to an appropriate specialist in your area.
After you complete the first step of submitting the Second
Opinion Consultation Request,
you will be asked to complete Step 2, Authorization of
payment.
Upon completion of the Authorization, our infertility
specialists will review your medical record.

Our Second Opinion is based exclusively on the information
transmitted to us electronically. The Second Opinion therefore can not be equated
to a full medical consultation which relies on a face-to-face encounter between
